How do I know if I qualify?
If your plan offers the Food & Home Card benefit, all 3 of these need to be true for you to qualify:
- You have one or more serious, ongoing health conditions from the list below
- You are at high risk of hospitalization or other serious health problems
- You need help managing and coordinating your care
1. Qualifying health conditions
For C-SNP members:
The qualifying health conditions for the Food & Home Card are the same as the conditions that qualify you for your C-SNP plan. These conditions are marked with an asterisk (*) below.
For all other plan members:
You may qualify based on any of the conditions listed below.
Conditions that affect your bones or joints:
- Arthritis, including rheumatoid arthritis or osteoarthritis
- Osteoporosis (weak bones)
Conditions that affect your heart or blood vessels:
- High blood pressure (hypertension)
- High cholesterol
- Cardiac arrhythmias, like atrial fibrillation (A-Fib)*
- Coronary artery disease*
- Congestive or chronic heart failure (CHF)*
- Peripheral vascular disease*
- Chronic venous thromboembolic disorder*
- Valvular heart disease*
Metabolic conditions:
- Diabetes*
- Severe obesity (Body Mass Index of 40 or higher)
Conditions that affect your lungs or breathing:
- Asthma
- Chronic obstructive pulmonary disease (COPD)
Conditions that affect your digestion:
- Crohn’s disease
- Colitis
- Inflammatory bowel disease (IBD)
Other qualifying conditions
- Chronic kidney disease (CKD)
- Any form of cancer, not including pre-cancer
- Alzheimer’s disease or another type of dementia
- Depression (major depressive disorder)
- Substance use disorder or alcohol/drug dependence
2. Risk of hospitalization or other serious health problems
To help us evaluate your risk, we look at:
- Your recent health history — for example, medications you take, recent doctor visits, your use of home healthcare services or medical equipment, and any recent falls or other health episodes
- Your life situation — including your everyday habits, finances, and how you’re managing your care and activities of daily living
- What your doctor tells us — like if your doctor fills out a form saying they think you qualify
3. Care coordination needs
To help us evaluate whether you may need help managing and coordinating your care, we look at:
- Your recent health history — for example, medications you take, recent doctor visits, your use of home healthcare services or medical equipment, and how you’re managing your chronic conditions
- Your life situation — including how you’re managing your care and activities of daily living
- What your doctor tells us — like if your doctor fills out a form saying they think you qualify
How does Devoted check if I qualify?
There are a few different ways we can confirm that you meet all 3 of these criteria:
- Claims and medical records: We check these automatically, as soon as they’re available to us. Many members qualify based on claims within just a week or two of enrolling.
- Health Risk Assessments: Your responses on this quick survey help us learn more about you and your health — and that can help us get you qualified for the Food & Home Card.
- Provider forms: Any healthcare provider who sees you can confirm that you qualify, whether they’re in network with Devoted or not. The form is available in the Devoted provider portal, or you can download a copy (form coming soon!). Note: If you are a C-SNP member, the same form that qualifies you for your C-SNP can also confirm your qualifying condition for the Food & Home Card.
The Food & Home Card is a special supplemental benefit offered on certain plans, and available only to chronically ill members with conditions like diabetes, chronic heart failure, cardiac arrhythmias, coronary artery disease, or peripheral vascular disease; qualifying conditions vary by plan. All applicable plan coverage criteria must be met and other conditions are eligible. Not all members qualify.
