What is a Devoted Medical Reward Card?

It's a prepaid card you earn when you complete qualifying health activities with Devoted Medical. We’re big believers in celebrating success, especially when it comes to taking care of yourself. Think of these cards as a small reward for putting your health first.

What can I use my card for?

You can use the card to buy gas and groceries.

How do I use the card?

• For gas, you have to pay inside ― you can't pay at the pump
• For groceries, you have to go to a grocery store like Publix, H-E-B, Kroger, or Safeway (no big box stores like Walmart or Target)
• Choose CREDIT at checkout — you don't need a PIN number
• If your purchase amount is more than what's on your card, ask the cashier to charge exactly the amount that's on the card — then use another payment method for the rest

When will I get my card?

We'll automatically send the card within 90 days of completing a qualifying activity. You don't have to fill out any paperwork or anything else. We'll mail the card to the address we have on file for you.

Do reward cards expire?

Yes. The expiration date is listed on the card in MM/YY format. It expires on the last day of the month listed. For example, “12/25” means the card expires on December 31, 2025.

Do I need a PIN?

Nope, just swipe your card and choose CREDIT when you pay.

How do I check my card balance?

Check your balance online. You’ll need to enter the card number and security code (the 3-digit number on the back of the card to the right of the signature box). You can also call us at 1-800-338-6833 (TTY 711) and we can check it for you.
