Devoted Health is an HMO and/or PPO plan with a Medicare contract. Our D-SNPs also have contracts with State Medicaid programs. Enrollment in our plans depends on contract renewal. To enroll in a Devoted Health plan you must meet certain eligibility requirements and reside in the plan's CMS-approved service area. Our plans’ 2025 service areas, as applicable, include select counties in Alabama, Arizona, Arkansas, Colorado, Florida, Georgia, Hawaii, Illinois, Indiana, Kentucky, Mississippi, Missouri, North Carolina, Ohio, Oregon, Pennsylvania, South Carolina, Tennessee, Texas, and Washington. Devoted Health complies with applicable Federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex. Representatives are available 8am to 8pm, Monday to Friday (from October 1 to March 31, 8am to 8pm, 7 days a week).
